I and/or those participating with me understand that yoga includes physical movements as well as an opportunity for relaxation, stress re-education and relief of muscular tension. As is the case with any physical activity, the risk of injury, even serious or disabling, is always present and cannot be entirely eliminated. If I/we experience any pain or discomfort, I/we will listen to my body and discontinue the activity. I/we assume full responsibility for any and all damages, which may incur through participation. Yoga is not a substitute for medical attention, examination, diagnosis or treatment. Yoga is not recommended and is not safe under certain medical conditions. By checking the box, I/we affirm that a licensed physician has verified my good health and physical condition to participate in such a fitness program. In addition, I/we will make the instructor aware of any medical conditions or physical limitations before class. If I am/we are pregnant, become pregnant or I am/we are postnatal or post-surgical, checking the box verifies that I/we have my physician's approval to participate. I/we also affirm that I/we alone am responsible to decide whether to practice yoga and participation is at my own risk. I/we hereby agree to irrevocably release and waive any claims that I/we have now or may have hereafter against teacher Bonita Orn and business Bliss with Bonita. Participation in a yoga session includes possible exposure to and illness from infectious diseases including but not limited to COVID-19, MRSA, and influenza. While Bonita Orn will never lead a session if showing symptoms, the risk of serious illness does exist. I/we knowingly and freely assume all risks, both known and unknown, and assume full responsibility for my participation. I/we willingly agree to comply with the stated and customary terms and conditions for participation as regards protection against infectious diseases. If, however, I/we observe and any unusual or significant hazard during my presence or participation, I/we will remove myself from participation. I/we, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS Bonita Orn and Bliss with Bonita with respect to any and all illness, disability, death, or loss or damage to person or property, to the fullest extent permitted by law. I/we have read and fully understand and agree to the above terms of this Liability Waiver Agreement. I am/we are in agreement voluntarily and recognize that checking the box serves as a complete and unconditional release of all liability to the greatest extent allowed by law in the State of California.
